It is important for patients to bring the following medical
records to the center at the time of initial visit, If these
records and scans are not available, the appointment may
need to be rescheduled, please make necessary arrangements
to have these tests taken prior to your arrival.
- Insurance card.
- X-rays, CT scan, MRI scan and the Radiologist reports
or readings from these scans. (Actual films must be provided;
no photocopy will be accepted)
- Bone Density Scan if you have been diagnosed with Osteoporosis
(photocopy is ok but it has to be readable)
- Results of previous testing or treatment (EMG's, physical
therapy, medical records, and/or if there is any corset,
brace or support prescribed for use, etc.)
- List of all the medications you currently take, and
a list of your drug allergies.

| You should take X-rays if
you have one of the following conditions: |
- You
feel persistent pain, numbness, or weakness
on part of your spine.
- You
have low back pain.
- You
have an accident and suffering pain on your
spine.
- You
have spinal fractures, dislocations, tumors,
bone spurs, or disc disease.
- You
have osteoporosis, arthritis of the spinal bones.
- You
have developed abnormal curvatures of the spine.
- You
have suspected or known to have a spinal defect.
- You
want to monitor changes in the spine after spinal
surgery.

| You should take CAT Scan
(CT) if you have one of the following conditions: |
- You have been diagnosed with spinal infection,
tumor, cancer or AIDS.
- You have internal bleeding caused by one of
the above conditions.
- You are suffering persistent pain from your
internal organs or abdomen.
- You have been diagnosed with spinal stenosis,
herniated disc, osteoporosis.
- You have developed abnormal curvatures of
the spine.
- You have suspected or known to have a spinal
defect.
- You want to monitor changes in the spine after
spinal surgery.

| You should take MRI Scan
if you have one of the following conditions: |
- You have been diagnosed with spinal infection,
tumor, cancer or AIDS.
- You are suffering persistent pain from your
internal organs or abdomen.
- You have been diagnosed with spinal cord injuries,
sciatica, osteoporosis.
- You have developed abnormal curvatures of
the spine.
- You have suspected or known to have a spinal
defect.
- You want to monitor changes in the spine after
spinal surgery.

| You should take Bone Density
Scan if you have one of the following conditions: |
- You have been diagnosed with osteoporosis
or osteoporotic kyphosis.

Upon arrival at Taiwan Spine Center, all personal and insurance
information will be verified. Patient will be asked to fill
out New Patient registration form
(it's recommended to download and complete the form before
you come to the center), once the registration is completed,
patients will be called in to the first available examination
room.
In the examination room, patients will be asked to change
into a gown and bathrobe. And the nurse will help to obtain
the height, weight and blood pressure about the patient.
Once these preparations are completed, our staff physician
will review medical history and records and also perform
a physical examination for the patient.
After the review of records and examination are complete,
each patient will receive an explanation of diagnosis and
recommendations for ongoing care, which may include non-operative
and operative treatments.
At the completion of the first visit, the clinic staff
will provide instructions for follow up procedures or appointments;
and give instructions on how to obtain additional information
or follow up on any questions or concerns.
The time necessary to complete an initial visit will take
about an hour. The complexity of patients' needs or emergency
situations may necessitate longer appointments or additions
to the clinic schedule.
